RE: Anti-Doping Acknowledgement Forms
The Anti-Doping Policy for the 2008 World Amateur Team Championships has now been posted on www.internationalgolffederation.org. Also included are the most current Prohibited List, a Player Guide to Anti-Doping, Penalty Table, etc.
Enclosed please find an acknowledgement form for each player to sign regarding this testing. Players who have not signed this form will NOT be permitted to compete. Please make enough copies so that each player from your country has their own form.
Minors must have the form completed by a parent or legal guardian. Additionally, where minors are concerned the form addresses consent to obtain a sample for anti-doping testing if selected, and medical treatment in case of emergency for any reason.
All players should have photo identification for proof of identity and age (e.g., passport, driver’s license, etc.) with them and readily available during the competitions.
The forms should be forwarded to your players and returned by 31 August 2008 to:
